Photo Membership Card
All SCoP membership cards will soon be required to include a photo of the SCoP member. No membership cards will be issued after January 1, 2013 without a member photo. As the office have limited scanning capability, we will accept only digital photos or passport style photos.
When submitting photos, please follow these guidelines:
- passport size
- Head and shoulders photo, in uniform
- If submitting a digital photo, submit in jpeg format
- Label your photo with name and registration #.
- A manager, facilitator or notary public must certify the photo is genuine and provide the member’s name and registration #.
If you are submitting photos of several members from an ambulance service or fire department, please submit them on a compact disk or flash drive and include a master list (first name, last name, registration #). The manager or facilitator must certify that the photos are genuine. This means we are asking the manager or facilitator to verify the identity of the person in each photo.
Individuals who are submitting photos and do not have a facilitator or manager can use a notary public to certify your photo.
