Reinstatement
Follow these steps for reinstatement in Saskatchewan:
- Download 2012 Registration Form (pdf)- This form must be completed in full and signed. The original form must be submitted. A copy or a fax will NOT be accepted.

Fees for 2012:
$275 reinstatement fee, plus the licensing fee of $435 (see below) for a total of $710. There is a pro-rated licensing fee as of July 1, 2011 of $348.00 plus a reinstatement fee of $275.00 for a total of $623..00. There is a pro-rated licensing fee as of October 1, 2011 of $261.00 plus a reinstatement fee of $275.00 for a total of $536.00.
Payment can be made by Visa, Mastercard, money order or certified cheque. Personal cheques will not be accepted.
Certificate and Transcripts. Evidence of successful completion of a CMA accredited training program or an equivalent program approved by Council. Download the Request for Transcript form (pdf) and have your institution forward transcripts directly to SCoP. Forward a photocopy of your certificate (a copy of your certificate may be on file).
If you have not practiced for two or more years, you may have to complete a Re-entry Program. Information on the program can be obtained from Norma Fellner at SIAST. Alternately, you must submit evidence of having met all registration requirements and completed all CME requirements. You may be required to write the Entry to Practice Licensing Exam.
- Criminal Records Check (CRC) including a vulnerable sector check (completed by a police officer within the past year). You must submit the original CRC. A copy will not be accepted.

Saskatchewan Jurisprudence. You will be required to sign a declaration that you understand the SK Legislation framework and protocols for your scope of practice based on reading the following - The Paramedics Act of Saskatchewan, Regulatory Bylaws, Administrative Bylaws, Emergency Treatment Protocols Manual and Continuing Medical Education Requirements.
Follow the link to obtain a copy of the Saskatchewan Jurisprudence Declaration. You must submit the original Jurisprudence Declaration. A copy will not be accepted. This declaration must be witnessed by someone other than a relative.
- If you are an EMT, PCP, EMT-P or ACP grad, then applicants must write a licensing exam. Please complete the licensing exam registration form and return it with payment to reserve your place.
- A photo (head and shoulders) . If you submit a digital photo in JPEG format, please label the jpeg file with your name and registry number (4587). The photo must be accompanied with a statement by a manager or notary public stating that you are the person in the photo.
