Licence Renewal form

To begin renewing your licence, sign in to Your Account and then go to Applications > Licence Renewal > Apply  (or just click that link after Oct 1)

All members must submit the Licence Renewal form. This form will ask you to answer basic questions about which type of licence you'd like (practicing or non-practicing), and allow you to attach supporting documents relating to your educational requirements.

The renewal form will:

  • Show you which certifications and skills have expired, and which are still valid
  • Require you to input the effective and expiry dates of updated certifications, skills and CME, plus upload proof of completion
  • And it will not allow you to submit until all fields are complete (but you can save your work and come back later)

HELPFUL TIP: If you are unable to meet your requirements,
you can choose a restricted or non-practicing licence while you recertify and save yourself potential late fees! Learn more about restricted and non-practicing licences »

Renewal Requirements

The Licence Renewal form will ask you to upload digital files which prove you have met the educational requirements. If you do not have access to a scanner, take a digital photo of each document and upload that (as long as we can read it). We prefer .jpg and .pdf files. Mobile uploads are ok!

Use this Skills Reporting Form for 2019 renewal - ACP to report skills. If you have a card or certificate, simply upload it when requested.

Need extra training or CME credits? Visit CME Opportunities for a list of training available.
